Required for: 

• • • Varsity athletes competing at invitationals that are permitted to provide their own transportation. 

    • Any athlete who is picked up from a meet and being driven home by another parent. I.e. An athlete rides the bus to the meet, and is permitted by THEIR parents to ride home from the meet with ANOTHER athlete's parent. This option requires a waiver signed by a DV principal to be signed by the athlete getting the ride, and the parent that is providing transportation must sign out their athlete at the meet, and sign out the athlete they are transporting. 

Not Required for: 

• Athletes riding the bus to/from a meet. 

• Athletes riding the bus to the meet, but getting a ride home from their parents. In this case, a waiver is not required, however, parents are required to sign out their athlete from the meet. 

• Athletes getting rides home from DV. If the athlete rides the bus to/from the meet and gets a ride home from DV with another parents or student, a waiver is not required. Only required FROM the meet. 

• Transportation waivers or sign out forms are not required for home meets. 

Other Important Notes: 

• A waiver is required for every meet that you will be providing your own transportation. One form for the entire season is not permitted this year. 

• A principal's signature is required for Athletes driving themselves, or athletes carpooling with another family where the adult is driving. 

• Principals are not available to sign waivers during spring break. If you did not get this signature prior to spring break, your only option for Red Mt Rampage and Chandler Rotary is to have parents drive or ride the bus. 

• Athletes are NOT allowed to drive another athlete to/from meets. There is no waiver for this situation. Exception: siblings 

• Do not ask Coaches at meets to make exceptions to the waiver policy. We can not break Tempe Union District policy.  Make your transportation plans ahead of time. 

• List of athletes that have turned in their waivers will be available for parents, athletes, coaches to view. Link provided on Parent/StudentSquare. 

• It is recommended that you take a picture of your waiver prior to turning into coaches. This can help expedite things if questions on the waiver come up or the waiver gets lost at some point.
